Reducing low density lipoprotein (LDL) cholesterol is the priority in managing patients with severe dyslipidaemia (cholesterol abnormality) and at high risk of cardiovascular (CV) death and disability. Yet even when treated with potent statins, many patients fail to achieve LDL-cholesterol targets and therefore their risk of accelerated atherosclerosis and CV death remains high. This problem is particularly critical in patients with familial hypercholesterolaemia (FH) (the most commonly occurring genetic condition in the world). People with FH typically have very high LDL cholesterol levels on treatment, and consequently, are at very high risk of premature and sudden CV death, myocardial infarction and stroke. Additionally, some patients are unable to tolerate statins – particularly at higher doses – and, therefore, do not achieve LDL-cholesterol goals. What is the importance of PCSK9? Discovered in 2003,1 PCSK9 is a protein made by the liver which increases the removal of LDL receptors from the surface of liver cells. These LDL receptors are critically important in removal of serum LDL-cholesterol from the circulation. People with so-called ‘gain of function’ PCSK9 mutations over-express PCSK9, and therefore have very high plasma levels of LDL cholesterol and are at very risk of atherosclerosis and CV mortality and morbidity.2 In contrast, people with so-called ‘loss of function’ PCSK9 mutations, have very LDL cholesterol levels and reduced risk for heart disease.3 What is PCSK9 inhibition? Different approaches to lowering PCSK9 are now licensed. These include fully human monoclonal antibodies (alirocumab, evolocumab) and a small-interfering RNA therapy (inclisiran). Trials show that these PCSK9 inhibitors reduce LDL cholesterol levels by about 50-60 percent - a significantly greater effect than that achieved with currently available therapies – either when given alone or in combination. These LDL cholesterol lowering effects have been consistently across a broad group of patients, including those with familial hypercholesterolaemia in clinical trials.4 Which clinical settings will benefit from the availability of PCSK9 inhibitors? These treatments offer the potential to reduce cardiovascular risk in patients whose clinical needs cannot be satisfied by current therapy. These include: Patients with familial hypercholesterolaemia – an inherited disorder of lipid metabolism with a very high risk of early CV death or morbidity, if not diagnosed or treated early Patients who cannot tolerate statins, who therefore remain at high risk of CV events such as heart attacks and strokes Patients with established heart disease who have not achieved LDL-cholesterol goals despite current therapies

Montrose Tennis Club is delighted by the faith shown by announcing a £23400 grant award from sportscotland – the national agency for sport. This investment, which is a 50 % grant of the money required to create two mini red courts, will complement the original six LTA standard courts already with upgraded surfaces and all floodlit. The grant ensures that the five year development plan to create a tennis centre of excellence is very much on track. The further money required has been fundraised by club members and the club’s sinking fund, as well as grants from Montrose Common Good, Aberbothock Skea Trust and Colin Grant Trust and any additional donations from local business would be appreciated too. The court build will commence after the club championships later in August. At the club, there is presently a great demand for tots tennis while the under 10 age groups continue to grow and we will now have an area to develop both their skills and play appropriate tournaments on. This new area will also be ideal for disability groups where they can focus in a smaller area and enjoy learning new eye to ball skills as well as holding tournaments. There is a definite spin-off from high profile tennis influenced by Andy Murray. Welcoming the funding Montrose Tennis Club’s secretary, Rhona Alston said “This development will continue to help Montrose Tennis Club, widen accessibility to the sport of tennis within the community and increase membership to the club. Now with the aid of full time coach Graeme Walker and Performance coach Kris Soutar plus invited coaches, there are programmes for all ages, ranging from three-year-olds to programmes for school pupils right up to veterans”. Mrs Alston continued: “Significantly, Montrose is well and truly on the tennis map, with three competing ladies teams, three men’s teams, four mixed doubles teams and the only club in Tayside to be playing out four junior teams as well as competing in junior Aegon fixtures all over Tayside and Aberdeenshire”. Last year was the first time the club were able to hold an official LTA tournament, reinstating the famous old “North Angus Tournament” now sponsored by Doug & Sandra Cree from D C Lighting Services Ltd, and it is on again this year 26th, 27th and 28th June. Entries are now open on LTA Tennis Scotland website and we are hopeful of attracting even more entries.

The Sapphire Project Organisation is an exciting new modern-day service for adults age 18 and over living with a learning disability. We want to understand what people want from their support, and to make sure that families and friends know how we will help them to make it happen. We offer a unique learning environment for our service users and although support needs may differ, we know that with the right support, people with learning disabilities can live full and meaningful lives. Aims Our aim is for service users to develop competence, respect, dignity, responsibility, independence and to promote their inclusion within the local working community. Working closely with service users we aim to find out what their passions are and wherever possible assist with overcoming challenges and removing barriers in order to enable them to live more fulfilling lives. What we offer Service users are offered the opportunity to develop transferable work skills, enterprise skills, independence skills, practical skills and social skills all within a variety of settings while being supported by qualified staff. Our project includes a café, a licensed bar and a kitchen garden all of which is open to the general public. We also offer a opportunities to develop general maintenance and wood working skills. Service users also have the opportunity to carry out a number of routine domestic tasks to help develop their independent living skills. These include, planning and shopping, cooking meals, basic housekeeping, getting out and about in the community and residential experiences. We support people in developing work and independence skills whilst having fun and making friends. Our aim is to break down barriers and to promote inclusion within the local community. What Makes Us Unique? We want to offer people the chance to learn valuable life skills along with work skills and our base at Hillsborough Arena provides us with numerous opportunities to achieve this objective. We offer an attractive, non-institutional environment with a team of committed staff with a very low rate of staff turnover, so people can develop strong, beneficial relationships over many years. We also offer a dedicated key worker to overlook and support the needs of both the service user along with the parent/carers. We want to understand what people want from their support, and make sure that families and friends know how we will help them to make it happen. Having worked in social care since college, Able Hands Together Founder Laura Tween focussed her work on adults with learning disabilities. After completing her law degree it soon became apparent that the average office environment wasn’t the space for her. She became a social care consultant, and it was through this work that she found her creative energy and passion for community projects was highly suited to being a social entrepreneur. Whilst delivering social care, Laura found that most therapeutic activities for people, like cinema or bowling, were entirely non-skills building. Laura wanted to create an offering where they could learn new skills, build their independence and work on emotional well-being simultaneously. A therapeutic farm service ticked all these boxes. Able Hands Together was established in 2015. The role of the Co-Farmer was soon born, to assist with day to day farm work, building employability, interpersonal skills, independence, and self esteem. Both staff and Co-Farmers work together to care for the animals, & plan and maintain the smallholding to make the most of the growing season. We have just had the exciting news that Able Hands Together has been approved as an Alternative Education Centre by Wiltshire County Council for Co-Farmers aged 18 and over. Laura is immensely proud of Able Hands Together and welcomes you to get in touch. Jason Rhodes Jason Rhodes Director Jason is the Managing Director of Commercial Business at Nottingham Trent University. Jason brings a wealth of business knowledge and advice to Able Hands Together. Jake Tween Jake Tween Director Jake interprets Government Policies on Education to ensure that the advice given by Able Hands Together is of a high standard. He also serves as a great consultant when it comes to the boring paperworky stuff. Nicolette Ann Shears-White Team Member Nic has worked at Able Hands Together for a number of years now and is proud to have a close working relationship with all the Co-Farmers. Having previously been a social worker, Nic is well connected with Hampshire based learning disability professionals and is very knowledgeable about safeguarding and rights. She promotes our Co-Farmers’ independence and works with a skills based approach with our clients. Carl Anderson Carl Anderson Team Member Carl is an ex-member of Her Majesty’s Armed Forces. He brings a wealth of skills-based services to the farm. If something needs to be built, Carl’s the man to do it! Our Co-Farmers love helping him to construct and build everything, from goat gates to raised vegetable beds. All staff that support our Co Farmers have DBS checks, are employed with their employment history and references checked. They are trained in safeguarding, Health and Safety and Equality and Diversity. Staff receive ongoing supervision and have access to further training.
